Fertility treatment can be grouped into three categories:

1. Medicines are given to improve the fertility rate of the couple. This can be used alone are in combination with any surgical procedure. Medicines can correct hormonal imbalance and brain in the ray of hope of the conception in the couple.

3. Assisted conception is a procedure wherein the counselling of the couple is done and then assistance in the conception process is suggested. Several techniques such as in vitro fertilization (IVF), gamete intrafallopian transfer (GIFT), intrauterine insemination (IUI) and intracytoplasmic sperm injection (ICSI) are suggested as per the requirement.

Ovulation is the process wherein the ovary produces the egg and on maturation it releases the egg. Each month the woman has go through this ovulation process. However this process continues till menopause only. Due to various reasons depending on the case, ovulation process gets halted and deviates from the normalcy. In such cases treatment is required for normal conception process. The ovulation process is controlled by the particular set of hormones called gonadotropin. These hormones are produced by the pituitary glands. This hormone will stimulate the working of the ovaries in the women and working of testes in men. The gaonaodotraopins are also called as the follicle-stimulating hormone (FSH) and luteinising hormone (LH). These hormones eventfully pass to the ovaries through the bloodstream.

In case the sperm oaf the male partner is abnormal then ICSI treatment is applicable. ICSI or intracytoplasmic Sperm injection is a procedure which is done in the hospital. In this procedure the sperm is injected to the egg. Once the sperm is fused with the egg the embryo develops. The good quality embryo is then transferred to the womb of the female partner. This is an advanced technique available in some of most advanced centres only. The remaining procedure and precautions to be taken remains the same as in the IVF technique.PESA / TESA:
Azoospermia is a condition wherein the sperm development is nil so, in such cases the sperm retrieval is done by minor surgical procedures. Percutaneous Epididymal Sperm Aspiration or Testicular Sperm Aspiration is a procedure which is carried out under mild sedation. A needle is used to pull out the sperms from epididymis or testicles.Instructions to the patient:

The patients are advised to be relaxed throughout the procedures and prior to coming to the hospital, it is advised to follow an overnight fasting. Hospital admission will be done by 7.30am. The expert will numb the area by inserting a needle. At araouand30 minutes of time is taken to complete the procedure. This is painless procedure so you can resume back to work again.


This is a sophisticated procedure wherein a hole is created in the outer lining of the embryo. This procedure helps in better implantation of the embryo in the uterus. This procedure is offered to the couple depending on the requirement and the diagnosis. This actually helps to enhance the success rate of the pregnancy. This procedure is carried out just before the embryo transfer.


In this procedure the embryo is allowed to develop till the formation of the blastocyst. Normally the embryo is transferred on the day 2 after egg collection however in this procedure the embryo is allowed to grow till day 5. The risk of the blastocyst transfer is more and can degrade the embryo growth hence losing a good quality embryo. However this procedure is done as required in a particular cases under medical supervision.


Embryo transfer is done as per the decision of the expert. However after the embryo transfer is done the remaining embryo will be assessed and stored at a freezing point. These frozen embryos can be transferred directly without going through an IVF cycle. The freezing process is not mandatory and is completely dependent on the couple’s choice. If the cycle is unsuccessful than the next FET (Frozen embryo transfer) cycle can be planned.

Instructions to the patient: Medication:

Once the cycle is planned than you will be given the IVF schedule. You will be given a oral contraceptive course one month prior to the cycle. Injections will start for down regulation prior to starting an OC pills. The first dose of the injection is given in the hospital. However the remaining injection can be given subcutaneously at your home. As per the schedule suggests you need to stop the contraceptive pills and just wait for your periods. On the 2nd or 3rd day of the cycle it is suggest to consume estrogen tablet to create a good lining of the womb.

Monitoring of cycle:

In this assessment the thickness of the endometrium is checked. For this you have to come down to hospital just once or twice. This is checked by undergoing trans-vaginal sonography repeatedly.

Embryo Transfer:

If everything is fine then the further step is the embryo transfer. The frozen embryo are then processed to get back to normal under the supervision of the embryologist. The quality and number is informed to you by the hospital staff. Later on the embryo transfer is done by a painless and simple procedure. So you can just walk down to the hospital with a stress free mindset. The only thing is you have to hold on the urine till the embryo transfer is done. The embryo transfer is done using thin needless cannula with no anaesthesia. You are offered with choice of Assisted Hatching. Later on you can just vacant your bladder.

Post ET care:

After the embryo transfer you can have care free time. The first few days you can take rest and there are no food restrictions. However supportive injections and medicines need to consumed for the next fifteen days.

Pregnancy Test:

Just after fifteen days of the embryo transfer a blood pregnancy test is to be done.


In case the couple has failed history of the IVF than they are suggested embryo donation option. Here there is no need to undergo repeated IVF procedure. The womb lining will be prepared by giving medications. And then the embryo transfer is done. This procedure is carried out in same manner as Frozen Embryo Transfer.


Process of IVF / ICSI cycle with Donor eggs and Embryo transfer in recipient:
This process is carried out only under the supervision of the fertility doctor. The couple`s permission is sought to take it further. For this procedure the detailed past medical history and surgical details are discussed. Once each and every detail is confirmed than the treatment plan starts with the donated eggs and the sperms. In this pre- treatment checklist the check ups are done on the partner and egg donors. They are prepared for the further treatment.

Selection of Egg donor:
The egg selection process is dependent on the couple. For this the donor profile as prepared and shared with you and later on depending on the choice the procedure starts within 4 to 6 weeks. The donor profile features the detailed medical history and features in it. The infections and baseline tests reports are also the added features mentioned in the donor profile. However the identification of the donor is not mentioned to maintain confidently.

Psychological evaluation:
This counselling of the egg donor is done. The egg donor is informed that they can continue the process of the egg donation till her choice.

Evaluation of the egg donor:
In this procedure thorough evaluation of the egg donor is done. Detailed screening tests are carried on the eggs of the egg donor. The infection tests for HIV, Hepatitis B and C, VDRL and CMV are done. Apart from this cytogenetic tests (karyotyping), Haemoglobin electrophoresis is done.

Cycle Coordination followed by Down Regulation:
The menstrual cycle of the donor and female partner is coordinated. Later on the down regulation injections (GnRH injections) are started on the donor first and after that the injection course starts in the female partner towards the 21st day of the period. The injections are given to both till one day before the egg collection process.

Stimulation of the ovaries:
The hormonal injections are given to the donor to stimulate the action of the egg production. The time period required is 10 to 11 days.

Travel to India:
Egg donation process is complex. So make your travel plans to India in advance. At least 2 to 3 days prior to donor`s egg collection you need to land in India.

Egg Collection:
The egg collection process is done ain an IVF theatre at around on the 14th day. The donor involvement is finished after this process. During the egg collection process the fresh samples are collected from male partner also. For this special arrangement are done in the hospital itself.

IVF/ICSI Laboratory Work:
Embryology laboratory is the pace wherein the eggs and sperms are fused. The embryo developed is then incubated in the special incubators. This is done till the embryo transfer process. The egg donor treatment is carried out till the embryo transfer procedure.

At the time of the embryo transfer what would be your preparation?
Cycle coordination followed by Down regulation
The procedure for the embryo transfer is your menstrual cycle and donor cycle is coordinated. The OC pills are given to you till the second day of the period to control the period date. This can be done by your GP. However the down regulation injections need to be taken on the day, the pill is stopped. After this you are likely to get the period. It is suggested to inform the hospital about the first day of the period. The injections are continued till the egg collection procedure.

Preparation the lining of the womb:
This is important process. For this hormonal tablet are given to the female partner. For this a hormonal tablet is given from the second day of the period to the daily basis. This helps in developing healthy lining of the womb. Healthy lining of the womb helps in better implantation of the embryo. The internal scan is fallowed on the seventh day to check the growth of the lining of the womb. These tablets are continued even after the pregnancy continues and later on they are reviewed for your fertility doctor.

Evaluation of the Surrogate:
The surrogacy requires a surrogate however to have healthy baby. Detailed screening of the health of the surrogate is done. The selected surrogate has to go through series of tests. Infection test screening is done for HIV, Hepatitis B and C. VDRL and CMV is also done. Cytogenetic tests (karyotyping), Haemoglobin electrophoresis are also done as part of screening.

Other supporting treatment to the surrogate mother:
Progesterone is a female hormone and it is given to the surrogate from the day of the egg collection. The courses of this hormonal tablet continue till full term pregnancy however later on this is reviewed as per the requirement.

Transfer of the embryos to the womb:
Actually 3 to5 days after egg collection, the embryo transfer is done on the womb of the surrogate mother. However for this the good quality embryo is selected and picked for implantation. Before the embryo transfer the embryo are graded for the goodness and quality and then they are shifted to the womb.

Back to Home:
The embryo transfer is a process done in the hospital however once this is done then the patient is send back to their home. However some prefer good rest in the hospital for first 5 days after the implantation. This means the abroad patients have to stay for 7 to 10 days in India.

Pregnancy Test:
This test is done on the 14th day of the embryo transfer. The blood tests are carried out to detect B HCG level in the surrogate mother`s blood. These tests are repeated 48 hours after this blood test.
